A final count of sponges, needles, and instruments is not considered a FINAL count until all items are accounted for and RETRIEVED.
Sometimes the team considers the final count "correct" just because they know where the missing sponge is with the plan of removal (e.g. 4x4 in the vaginal vault or laparotomy pad in the abdomen). A count should be considered final AFTER the item is removed and retrieved.
