Patient Services
If you are a patient of Cedars-Sinai Health Associates, our member service representatives act as liaisons regarding issues on health plan eligibility or enrollment, covered services and quality.
If you are a new patient with HMO insurance, we can assist you in finding a physician and registering with Cedars-Sinai Medical Group or Cedars-Sinai Health Associates. When you contact us, please provide:
- Full name
- Date of birth
- Insurance identification number
- The health plan you are with
- The person who has the insurance (self, parent, guardian or spouse)
- Any dependents
- Your address and telephone numbers (work and home)
