Community Service Forms
We are happy to fill out your community service forms for your participation in our Teen Volunteer Program. Please be sure to give us at least 3-5 business days to send the forms back to you. You can drop off your forms at the Volunteer Services Office or email them to firstname.lastname@example.org. Please note we cannot provide evaluations of your performance if your assignment was not in the Volunteer Services Office. We encourage you to ask your direct supervisor if you need an evaluation.
We understand you may need to miss your shift sometimes. Remember to follow proper protocol for dropping your assignment. This includes logging into your online account to cancel your shift, informing your shift mentors, and, if you are in a clerical area, informing your direct supervisor. You can cancel your shift online, using your credentials to log in.
To help ensure the health and safety of our patients, visitors and employees, Cedars-Sinai implements a flu vaccination policy for volunteers every flu season. Flu season begins Nov. 1 and ends on March 31. All volunteers must submit proof of vaccination before Oct. 16. Please note you will only be eligible to volunteer in our Summer Session should you decline the vaccine.
