Getting Started

Facing a cancer diagnosis is not easy. You’re not alone. We're here to help you understand your choices and support you throughout your care.

Your First Visit

Listed below are standard items to bring to your first doctor visit. Ask your doctor's office what else you need to bring with you.

  • Your driver's license or ID card
  • Health plan card
  • If needed, a paper copy of your referral or HMO authorization (approval). This can also be sent to your doctor before your visit.
  • Health history and physical form (PDF)  
    • Bring the filled-out form with you if you received one in the mail.
    • Or you can fill out the form when you meet with your doctor.
  • Any health records (such as X-rays, medical charts, lab tests) needed for your first meeting — these may differ from office to office and based on your symptoms or diagnosis
  • A list of all your medications and any allergies to medications
  • A list of questions
  • A family member or close friend to help you take notes
  • A notebook to record tests, procedures and treatments
  • if you need a translator, tell your doctor's office at least 24 hours before your visit
