History: 52-year-old male with left-sided facial pain for 3 years. MRI was performed when medical treatment for trigeminal neuralgia did not diminish the patient’s pain or discomfort.

T2 CISS MR image of the cerebellopontine angle demonstrates a tangle of flow voids on the left side in the location of the trigeminal nerve. Further evaluation was necessary for characterization.
