Hysterosalpingogram shows a broad fundal indentation. The differential includes septate uterus, arcuate uterus, bicornuate uterus, and large fundal fibroid. Two fallopian tubes are opacified from a single endometrial cavity making, excluding uterus didelphys.

MRI obtained for further evaluation of uterine abnormality seen on hysterosalpingogram.
