CT findings are most consistent with xanthogranulomatous pyelonephritis.

Hospital Course:

Patient was admitted to the hospital for management of sepsis due to UTI and medical optimization prior to surgical management.

After medical optimization, patient underwent surgical resection of the right kidney. Surgical specimen were sent to pathology for definite pathologic diagnosis.

DIAGNOSIS:
1. Nephrolithiasis with features of obstructive uropathy
2. Xanthogranulomatous pyelonephritis with acute pyelonephritis and focal abscess formation
3. Mild arteriolar nephrosclerosis
