Scheduling an Appointment

Thank you for choosing Cedars-Sinai. The Medical Genetics Program is proud of the team approach we take to ensure a well-rounded and thorough visit for all of our patients and their families.

Our team is led by board-certified medical ceneticists. Board-certified genetic counselors, social workers, dietitians, fellows and an experienced office staff round out our dedicated team.

Each new patient is asked to complete a new patient questionnaire before an appointment is scheduled. The completed questionnaire is both necessary and helpful to our staff in preparing for your visit. Please make sure to take the time to fill out the questionnaire completely and to the best of your ability. To ensure your privacy we ask that you fax or mail the completed forms to us directly. Once received, the packet will be reviewed and you will be contacted to set up an appointment within one business day.

Please select the appropriate packet by clicking on the link below. If you have any questions or wish to speak to someone in our office, please call us at 310-423-9914 or email us by clicking the contact us link in the left-hand column of the page.

Completed packets can faxed directly to a schedule at 310-423-2080 or mailed to the Medical Genetics Program, Scheduling Office, 116 N. Robertson Blvd., Suite 400, Los Angeles, CA 90048.
